Pan London Suspected Cancer Referral Forms

The Transforming Cancer Services Team has produced Pan-London Suspected Cancer Referral Forms.

These replace the suspected cancer referral forms produced by the former cancer networks.

The forms, educational resources and support information are available from Healthy London Partnership.

The new electronic referral forms are available on EmisWeb, Vision, SystmOne, DXS and available in a Word format. GPs in all CCGs across London are advised to use these forms when referring patients with a suspected cancer, as they contain the most up-to-date clinical criteria.

Practices in Central London, Hounslow, Ealing, West London, Hammersmith and Fulham, Lewisham, Greenwich, Bexley, Sutton and Islington CCGs have local arrangements in place. You should be contacted by your CCG cancer or IT lead who will advise you on how to access the new forms.

London CCGs, cancer/GP leads and Macmillan GP advisors have worked alongside hospital specialists, pathway groups and patient representatives to develop these forms. Our aim is to improve the pathways and referral procedures for patients with suspected cancer and we hope the introduction of these forms will be seen as a significant step forward.

Patient Information on Urgent Referrals

NHS England has produced information sheets for patients that can be downloaded and printed.

The information sheets explain to patients why have been urgently referred to hospital by their GP or dentist, what it means, what they need to do and what might happen next.

 

Pilot to test integrated community palliative MAAR charts in Harrow

 RM Partners is working with partners in north west London to pilot and test new integrated community palliative medicines authorisation and administration record (MAAR) charts in Harrow.

All patients discharged from Northwick Park Hospital in the last phase of life who require (or are anticipated to shortly require) medication to be administered by nurses in the community, will leave hospital with the drugs and an MAAR chart, which can be used by staff in London Northwest Healthcare University Trust (LNWH), Central London Community Healthcare (CLCH), and by GPs in Harrow.
